RGA-2024-DLTSS-01-INTEN: Intensive Treatment Services Housing and Homelessness InitiativeDivision of Long Term Supports and Services (DLTSS)

The New Hampshire Department of Health and Human Services, through its Division of Long-Term Services and Supports (“Department”), and its Division for Behavioral Health, is seeking responses to this Request for Grant Applications (“RGA” or “solicitation”) from qualified provider organizations to purchase, renovate and/or construct housing and/or increase capacity at existing homes for individuals who are eligible for one of New Hampshire’s Home and Community Based 1915c Waivers, or for individuals or families that have an individual who is disabled, and are experiencing homelessness.

All real estate purchases, construction, renovation and capacity expansion must be compliant with the Centers for Medicare & Medicaid Services’ Home and Community-Based Setting Requirements for Community First Choice and Home and Community-Based Services (“HCBS”) Waivers rules.
