The New Hampshire Department of Health and Human Services, Division for Behavioral Health, Bureau of Homeless Services the (“Department”) is seeking responses to this Request for Applications (RFA) from qualified Vendor(s) to provide housing assistance and supportive services for income-eligible individuals living with HIV/AIDS, and their families, to establish or better maintain a stable living environment in housing that is decent, safe, and affordable, to reduce the risk of homelessness, and to improve access to health care and supportive services.

The Department anticipates awarding no more than two (2) contract(s) for the services in this solicitation to cover the Balance of State and Manchester geography for the Continuum of Care.
