The New Hampshire Department of Health and Human Services, Division of Long Term Services and Supports (“Department”) is seeking responses to this Request for Proposals (“RFP” or “solicitation”) from New Hampshire counties to pilot a program that defers and delays institutional care for individuals who are dually eligible for Medicare and Medicaid benefits. The purpose of this pilot is to identify opportunities to develop a comprehensive community-based care system that supports individuals staying in the community longer, as opposed to entering institutional care.
